Credit card authorization forms can be filled online. Alternatively, customers can take print outs of the forms and fill them up by using a dark pen. It is advisable to fill up the print out forms legibly.
The person owning the credit card needs to put his signature on the designated place in the form.
The cardholder is required to provide a xerox copy of both sides of the signed credit card. The cardholder is then required to e-mail, fax or scan the duly completed form with a xerox copy of the credit card to the service provider.
Some of the items included in credit card authorization forms are nature of credit card, credit card number, expiration date of the credit card, VID code, billing address of the concerned credit card, the shipping address requested for, and such other items.
After this, comes the signature part of the credit card holder. For the online pro forma, the credit card owner needs to specify the validity period of the credit card also.
The customer can add the new address as the authorized alternative shipping address to his account
